My/our credit sale application will be submitted to Adventure Trikes for consideration as to whether a retail installment contract with me/us would meet its purchase requirements.  I/we hereby acknowledge notification that a negative report reflecting my/our credit record may be submitted to a credit reporting agency if I/we fail to fulfill the terms of my/our credit obligations.  NOTICE TO MARRIED APPLICANTS RESIDING IN WISCONSIN:  No provision of any marital property agreement, unilateral statement under s.766.59 Wis. Stats., or court decree under s.766.60 adversely affects the interest of the creditor unless the creditor, prior to the time the credit is granted or and open end credit plan is entered into, is furnished a copy of the agreement, statement, or decree or has actual knowledge of the adverse provision.  NOTICE TO ALL OHIO RESIDENTS:  The Ohio laws against discrimination require that all creditors make credit equally available to all credit worthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request.  The Ohio Civil Rights Commission administers compliance with this law.  I/WE AFFIRM THAT ALL THE STATEMENTS MADE IN THIS APPLICATION ARE TRUE AND CORRECT, THAT WE ARE 18 YEARS OF AGE OR OLDER, AND THAT THESE STATEMENTS ARE MADE FOR THE PURPOSE OF OBTAINING CREDIT.  THE CREDITOR IS AUTHORIZED TO OBTAIN CREDIT REPORTS ON ME/US TO INVESTIGATE MY/OUR CREDIT RECORD AT THE TIME OF MY/OUR APPLICATION AND FROM TIME TO TIME THEREAFTER TO CONFIRM MY/OUR CREDIT RECORD AS LONG AS MY/OUR ACCOUNT OR LOAN REMAINS OPEN OR IS CLOSED WITH AN UNPAID BALANCE.  THE CREDITOR IS ALSO AUTHORIZED TO VERIFY MY/OUR CREDIT, EMPLOYMENT, AND INCOME REFERENCES, AND TO OBTAIN SUCH OTHER INFORMATION AS THE CREDITOR DEEMS IN CONNECTION WITH MY/OUR APPLICATION AND THE PERIODIC REVIEW OF MY/OUR ACCOUNT OR LOAN.  I/WE ALSO AGREE THAT IN ADDITION TO REVIEWING MY/OUR APPLICATION AND ACCOUNT OR LOAN, THE CREDITOR MAY USE THE CREDIT REPORTS AND OTHER INFORMATION FOR OTHER PURPOSES, INCLUDING CONSIDERING ME/US FOR ADDITIONAL PRODUCTS AND SERVICES THAT ARE OFFERED BY THE CREDITOR DIRECTLY OR BY THE CREDITORS AFFILIATES.  THE CREDITOR ALSO IS AUTHORIZED TO GIVE CONSUMER REPORTING AGENCIES AND OTHERS INFORMATION REGARDING THE CREDITORS CREDIT EXPERIENCE WITH ME/US.  UPON MY/OUR REQUEST, YOU WILL INFORM ME/US OF THE NAME AND ADDRESS OF EACH CONSUMER REPORTING AGENCY FROM WHICH YOU OBTAINED A CONSUMER REPORT RELATING TO ME/US.
